Meridian Health’s Autism Care Coordination Program (ACCP)

Meridian Health

About our Medical Home

Improving the quality of and access to services for children 0-6 years with autism spectrum disorder (ASD) and their families by providing a comprehensive team approach and educating pediatric primary care practices (PCPs) throughout Monmouth and Ocean counties that care for children with ASD.

The ACCP is built upon the successful foundation of Meridian’s Infant Toddler Assessment Program (ITAP) that was previously implemented at Meridian’s K. Hovnanian Children’s Hospital at Jersey Shore University Medical Center (JSUMC) with funding from the NJ Governor’s Council for Medical Research and Treatment of Autism. The program has been successful in increasing the number of children who receive earlier autism diagnoses by implementing a team secondary screening program to facilitate early identification and diagnosis, as well as by increasing local pediatric primary care practices’ understanding of autism and helping them incorporate the M-CHAT (Modified Checklist for Autism in Toddlers) screening tool.

This pilot is aimed at improving the quality of and access to services for children 0-6 years with autism spectrum disorder (ASD) and their families by providing a comprehensive team approach to follow-up visits for patients diagnosed with ASD at Meridian’s Child Evaluation Center at Jersey Shore University Medical Center as well as providing education to, and working closely with, pediatric primary care practices (PCPs) throughout Monmouth and Ocean counties that care for children with ASD.
