About our Medical Home

In partnership with key stakeholders, we will create an evidence-driven care transition tool to address transition needs of teens with ASD, integrate the tool into practice, and provide education in the Special Needs Primary Care Medical Home in Mountainside, NJ.

Youth with ASD face many issues as they transition to adulthood. Transition affects education, employment, relationships, mental health, financial planning, daily living, decision making and more. Transition needs require greater attention and standardized approaches. Building upon our existing practices and continuing with the Autism Medical Home care coordination initiative, we plan to improve care for youth with ASD by creating and implementing a comprehensive, standardized, evidence-based transition tool. By introducing a transition tool, we would like to standardize best practice. By ensuring that the tool is efficient and practical, we focus on the most salient transition needs and offer replicable results for community providers.

With the amount of patients in our practice requiring transition services, this initiative provides us the opportunity to truly improve their care. By solidifying a transition tool, we would like to build advocacy skills for the patient and their family so they feel more prepared for transition to adult providers. Similarly, we would like to build resources and knowledge for our physicians and staff so that they feel confident in addressing transition needs. The Practice hopes to see a smoother, successful transition from pediatric to adult healthcare.
